- Provide employment free from recognized caught-between hazards
- Guard points of operation of machinery
- Ensure devices adequately protected machine operators
13 serious violations, including failure to:
- Train employees on noise hazards
- Ensure employees wore eye, foot, and hearing protection
- Take unsafe powered industrial trucks out of service
- Ensure containers of hazardous chemicals were properly labeled
